Depression Checklist

If you are concerned that you may be depressed, use this symptom checklist to help you determine if you are experiencing depression.

Over the last 2 weeks, have you been bothered by any of the following problems?

___ Little interest or pleasure in doing things

___ Feeling down, depressed, or hopeless

____ Feeling more irritable or angry than usual

___ Crying more often

___ Trouble falling or staying asleep, or sleeping too much

___ Feeling tired or having little energy

___ Poor appetite or overeating

___ Feeling bad about yourself—or that you are a failure or have let yourself or your family down

___ Trouble concentrating on things, such as reading the newspaper or watching television

___ Moving or speaking so slowly that other people could have noticed OR being more fidgety or restless than usual

___ Thoughts that you would be better off dead, or of hurting yourself in some way
